How We Assist Your Practice
- The Health Choice Pathway Provider Portal is designed with you in mind. Included in this site, and accessed through a secure portal, is patient data such as claims history and prior authorizations.
- We work closely with your team to streamline and expedite prior authorization by minimizing the number of procedures requiring prior authorization. Many of the items on our abbreviated prior authorization list ask for notification only.
- Customer service is the fabric of our organization. Our internal infrastructure is designed to provide your patients with speedy access to customer service representatives and to expedite claims processing.

Once the Request for Participation/Update Provider Information Form is received by the Health Choice Pathway Network Services Department, we will evaluate and compare our membership needs to current network availability, along with several other criteria, which include, but are not limited to: board certification, languages spoken and office locations.
Once the previous items have been reviewed, the request moves to the Health Choice Pathway Contracting Committee for review. If approved, the Network Services Department will contact you to initiate the Contracting and Credentialing process. Health Choice Pathway credentials providers according to nationally recognized standards that serve as the model for minimum credentialing requirements. We do not discriminate against age, color, creed, disability, national origin, race, religion, sex, or sexual orientation.
Credentialing generally takes about 60 days from our receipt of a complete credentialing application and up-to-date attachments.
